  1. No I would not bother removing shingles to install ice and water shield on a roof over. It might telegraph through where the roof changes from a single layer to a double layer. I guess you "could" if you wanted to but imo its a waste.
  2. Its really up to you what kind of valley you prefer. Closed cut or open metal valley (you would have to add new metal on top of your current shingles for this) would both do the job just fine.
  3. If I were you I would just keep the box vent system (but not reuse the old ones, get new). Again its something you "could" do but if a customer asked me I would inform them if they want woodwork done and new ice and watershield installed they should do the job right and not go with a roof over. But if you really wanna plug the holes you remove the vents, add some 2x4 blocks under the holes then plug the holes with some plywood nailed into the 2x4.
